I designed a print and digital directory for the Healthy Ageing at Home service, an initiative by the South West Sydney Primary Health Network (SWSPHN). This project aimed to assist older individuals with chronic health conditions in navigating resources to enhance their health and wellbeing while living independently. The directory was created to be user-friendly and accessible.
